You should not comment on how they look. Most of these persons are already aware of their weight and they do not want to be reminded again of how they look. You should also not try to compliment them, as well, since they will only be more obsessed with their body image and their weight.

You should never demand that they change almost immediately. If you do, they will try to rebel and will not try to help themselves. This change may take time and should be done on their own terms. If they try to change immediately, it may cause adverse effects on their health, instead.

You should be encouraging him to manage his emotions in healthy ways. You should be giving encouragements to the individual as he is needing them the most. He will be motivated to change on his own and have himself cured of this sickness.
